Affidavit (NVL-003)
Used to reinstate a vehicle registration which has been suspended for failure to maintain
liability insurance. Only used if the vehicle has not been operated during the lapse of insurance.
This form is for use only if the vehicle was last
titled in Nevada.
If you are not sure whether your vehicle is titled in Nevada or if you do
not have all of the information requested, please contact the Records Section for instructions.
If your vehicle is titled in another state, you must obtain a duplicate
from that state.
You must file this form within 10 days of any accident which was: (1) not investigated at
the scene by law enforcement and (2) caused injury, death or more than $750 damage.
